Tammy Hart Cook, 54, of Madison, died Thursday, March 7, 2013, at Norton Hospital in Louisville, Ky.
She was born Dec. 7, 1958, in Madison, the daughter of Raymond Fay Hart Sr. and Shirley Haskell. She graduated from Madison Consolidated High School in 1977 and received a social work degree from Ball State University. She was a social worker, having worked in Seymour and then 10 years at the Muscatatuck State Development in Butlerville. She also was a social worker and group home administrator at Crest Services in Des Moines, Iowa for 20 years before returning to Madison in 1994 to care for her mother. She was a member of Lutheran Church of Hope in Des Moines, Iowa and then North Madison Christian Church. She enjoyed shopping, traveling, her animals, socializing with family and friends and working out.
She is survived by one son, Tyler Cook of Johnston, Iowa; her father of Hanover; seven sisters, Bonnie Hart and Gina Jenkins, both of Madison, Traci Peddie of Indianapolis, Brandy Graham of Madison, Mindi Brent of Campbellsburg, Ky., and Carrie Hart and Jayme Hart, both of Madison; one brother, Raymond F. Hart Jr. of Chicago, Ill.; her grandmother Nellie Johnson of Elwood; 11 nieces and nephews; two uncles; and several cousins.
She was preceded in death by her mother; her grandparents, Robert Hart, Herbert W. Haskell and Geraldine Lauderbaugh Haskell; and one uncle.
